Creosote builds up every time you burn. Enough of it, and a stray spark in the flue turns into a chimney fire. We sweep the full length of the flue, the smoke shelf and the firebox, and we do it with drop cloths down and a HEPA vacuum running.

Once a year for anything you burn regularly, or after about 80 fires — whichever comes first. If you burn unseasoned wood, or you run a wood stove as your main heat source, you may need it twice a season. Gas fireplaces still need an annual inspection even though they produce almost no creosote.
A straightforward single-flue sweep and Level 1 inspection is usually about an hour. Add time if there are multiple flues, heavy glazed creosote, or an animal nest to remove.

When a house is being bought or sold, after a chimney fire or any event that could have damaged the structure, and any time you change what the chimney vents — a new liner, a new appliance, or a switch from wood to gas.
Puffy or honeycombed creosote, discolored or warped damper parts, cracked flue tiles, and sometimes creosote flakes on the roof or in the yard. Plenty of chimney fires burn out on their own without the homeowner ever knowing, and leave damage behind that only shows up on an inspection.
Yes. Gas fireplaces and their vents still need annual inspection and cleaning — debris, corrosion and blocked vents are all real problems, and a blocked gas flue is a carbon monoxide risk.
